Specialized Residential Homes

We all want the best for our loved ones.  We want to keep them close, be there for them when care is needed, and help them live life to the fullest.  But there are times when we may not have the means to provide for family members or friends with unique physical, medical or behavioral needs.

Specialized-Residential-Homes2That’s when individuals with intellectual and developmental disabilities and their families turn to California MENTOR and our partners—Loyd’s Liberty Homes and Cornerstone Living Skills—for our community-based specialized residential support services.

We work with California’s  Regional Centers to ensure that our residences are tailored to fit the needs of each person who calls them home—from individuals who need support with certain daily activities to those coming out of institutions requiring 24/7 care.  And family members and friends can rest assured that their loved ones are receiving expert clinical supports and compassionate residential care from our experienced team of health and human service professionals.

Intermediate Care Facilities

Loyd’s Liberty Homes and Cornerstone Living Skills—both partners of California MENTOR—offer Intermediate Care Facilities (ICFs) with 24-hour medical care and support for adults with developmental disabilities throughout the state.  Our community-based ICFs—otherwise known as group homes—allow individuals to feel comfortable, safe and supported.  Each of our ICFs specializes in a different level of care and enables individuals to receive the supports they need in a place they can truly call home, including:

  • Personalized support to help individuals become more independent.
  • Specialized services for individuals who have physical or behavioral challenges.
  • 24/7 nursing and treatment services for people with intensive medical needs.

We get to know each person in our ICFs and develop Individualized Service Plans that meet the needs of each individual.  Our ICFs also offer 24/7 emergency support from a physician and a psychiatrist.

Specialized Homes and Nursing Services

SRH-ProgramWe also offer more intensive support for adults in California with developmental disabilities and complex medical challenges through our Specialized Homes—known to some in the state as 962/853 Homes.  Each location is staffed with a registered nurse, a licensed vocational nurse, and a certified nurse’s assistant—who provide 24-hour nursing support to ensure the comfort and well-being of every individual.

SRH-Program2Nestled in communities throughout the state, our specialized group homes feature state-of-the-art equipment—such as lift-tracking systems—that helps support those who require physical assistance.

The adults in our 962/853 homes have Individual Service Plans and Individual Health Care Plans that keep the focus on personalized goals.  We offer the following specialized health services in our intimate homes that support up to five individuals:

  • Occupational therapy
  • Physical therapy
  • Recreational therapy
  • Respiratory therapy
  • Nutritional support
  • Kidney dialysis
  • Special medication regimes
  • Pain management

Active, Community-based Care

CA-MENTOR-Agnews(1)California MENTOR has a track record of offering positive, engaging experiences that set us apart from other residential programs.  We recently transitioned 50 individuals from the Agnews Developmental Center when it closed, and successfully took them into the community and created an environment where they are now thriving.  We encourage individuals to be a part of the community.  No matter the intellectual or developmental disability, our staff accompanies individuals on walks, trips to nearby cities and landmarks, and other engaging community events.
